Person Centred Practice

Person Centred Practice are ways of commissioning, providing and organising services rooted in listening to what people want, to help them live in their communities as they choose.

      Here are some examples of what you would see where there is person centred practice:

  • Person’s perspective is listened to and honored
  • Individuals have a role in planning the supports they receive & the staff that are hired
  • Regularly look at people’s lives to see what is working and not working Employees know their roles and responsibilities in supporting people
  • Staff are matched with people based on skill and common interests
