Families and Children

Person centred thinking and planning are being used in nurseries and schools and throughout the transition process. We are working with 6 schools to develop best practice in using person centred thinking and planning and develop one page profiles. Our understanding of best practice to date is described in the book “Person Centred Approaches in Schools”

There is a national programme to roll out person centred transition reviews in year 9 and year 10.

We worked for two years with Sure Start to develop person centred approaches.

When, for whatever reason, people are not able to lead the person centred plan themselves, family members are in a great position to develop a plan with and for their family member. Over the last 3 years there has been a range of learning opportunities developed for and with families around planning. They are now starting to be delivered and families are beginning to support each other and share their learning. Services are realising that they will have to start to respond to plans that families develop. People are learning together.
