This workshop also looks at how the One Page Profile can be used to direct what happens with supports and services. In essence, this workshop provides a method that ensures that planning is done in a person directed manner.
Family members and caregivers develop real plans that can be used as the foundation of a person's Support Plan, Individual Education Plan or other planning documents used by service providers.
Who is it for?
This workshop is designed specifically for family members and their loved one who uses support services.
The person and their family can invite other family members, friends, Service Coordinators and Support Providers to attend as members of their team.
What will people know and be able to do as a result of
attending this course?
Use the core person centred thinking tools and be able to:
• Understanding the values and benefits of person centred and person directed planning
• Gathering information from the person to discover what is 'Important To' and 'Important For' him or her
• Gathering information from other perspectives (other family members, support workers, etc) about what is 'Important To' and 'Important For' the person and how to support him or her in staying healthy and safe while living an active life in the community
• Learning to be the leader in creating a person centred plan and using the plan to direct services and supports
Optional follow-up sessions Follow-up sessions are
available.

• Working/not working
• Relationship map
• Communication charts
• Doughnut sort
• Learning logs
• Matching supports
• Presence to contribution
• Hopes and dreams
• Decision making agreements

This workshop is designed to be lead by a person or a small group of people who have a family member who receives supports from a service organisation (developmental/learning disability services, older people, mental health, etc).
The first step to becoming a Families Planning Together trainer is to attend this workshop.
