Pathways and Profiles: Using person centred thinking tools with the Getting a Life transition pathways

The Getting a Life Pathways covering employment, health, becoming independent (home) and friends and community show us how we should be supporting young people through the transition from school to a fulfilling adult life. Person centred thinking tools are invaluable in helping us do this well.

During this two day course we will:

  • Share eight person centred thinking tools that are especially relevant to young people to develop one page profiles and embed them into every day life.
  • Explore how the information gathered could be used as a foundation for the single assessment process or 'Education, Health and Care Plan' (SEN green paper: Support and Aspiration 2011).
  • Understand, from the Getting a Life pathways, what needs to happen to ensure a good transition for the young person and their family.

 

Who is it for?

Anyone working in, or connected to, the school environment; teachers, teaching assistants, SENCos, connexions, volunteers, health professionals, social workers, care managers, reviewing officers.

What will people know and be able to do as a result of attending this 2 day course?

Use a range of person centred thinking tools, that are particularly helpful during transition and in every day work, such as:

  • Separating important to someone, from what is important for their health and safety.
  • Working/not working from different perspectives.
  • Decision making agreements.
  • Communication charts and learning logs.
  • Presence to contribution.
  • Relationship circles and capacity mapping.

Next Steps?

As well as giving participants the opportunity to learn about a wide range of person centred thinking skills, this course is the foundation for building the core skills required to facilitate person centred reviews. Some people attending this course may wish to consider attending the 2 day Person Centred Reviews in Transition course to go on to develop their review facilitation skills.
