There are ways of facilitating reviews that puts the person at
the centre of the process and draws on person centred thinking
tools and approaches. This process often does not take any more
time or preparation, but will develop a clear and focused person
centred action plan with the person.
While the process was designed with transition in mind, they are also being used with adults, children and young people in a range of different service situations. This course develops people's skills to enable them to facilitate two types of person centred review.

This three day course (with one day follow-up) gives people:
This course can be offered specifically as person centred thinking with older people, people who use mental health services, learning disability services, physical disability services, long term health conditions, end of life, people with dementia, people who use drug and alcohol services, children and young people. The person centred review process is often used at the year 9 transition review meeting in schools.
Anyone responsible for facilitating reviews, including; care managers, social workers, reviewing officers, health professionals, managers.
