Person Centred Reviews in Transition: Using the Transition Pathways

This is for people who have previously attended the Pathways and Profiles course who wish to build on those foundations to facilitate reviews.

The first review a young person would attend is the important to/for review, typically in Year 9 and this has been strengthened to include employment, health, becoming independent (home) and friends and community. The second review, usually a year later, would build on this further to share and action plan the postive and possible ideas for the future.

 

This two day course will:

  • Enable you to facilitate two types of review in transition: the Important to/for review and the What's possible review
  • Understand from the Getting a Life Pathways what needs to happen to ensure a good transition for the young person and their family
  • Set the foundations for Year 11 reviews where young people will be planning with a predictive personal budget.

 

Who is it for?

This is for people who have previously attended the Pathways and Profiles course who wish to build on those foundations to facilitate reviews. This is likely to be anyone working in, or connected to, the school environment; teachers, teaching assistants, SENCos, Connexions, volunteers, health professionals, social workers, care managers and reviewing officers who facilitate reviews as part of their role.

 

What will people know and be able to do as a result of attending this 2 day course?

  • Facilitate important to/for reviews incorporating the Getting a Life Pathways
  • Facilitate what's possible reviews focused on what is positive and possible around; employment, being independent, health, friends and community
  • Further increase and practice a range of person centred skills to add to the ones already practiced on the Pathways and Profiles course, such as blue sky thinking and three plus three
