Re-ablement, the New Journey. Design Day.
This is a one day workshop to design and map out what is needed locally to be able to offer a new reablement journey that complements your local vision.
What does this day cover?
• Introduce the New Reablement Journey
• Agree your definition of reablement and supporting principles
• Evaluate where you are now in relation to reablement
• Introduce the Self Assessment checklist for reablement
• Develop a project plan/proposal
The objectives for this day are for participants to:
- Be introduced to the vision and principles that support the New Reablement Journey
- To create a shared understanding of what a successful reablement process looks like locally
- To clarify what is working and what is not working about your current reablement processes, protocol, documentation and journey
- Develop a detailed understanding of what we are building on and what needs to change, using the Self-Assessment Checklist
- To have a detailed understandfing of what we are building on and what needs to change, using the self-assessment
- Develop a high level action plan

Anyone involved in the design and delivery of Reablement, Intermediate Care Services and Self-Directed Support. Manager's. Senior Management and front line workers.
