Working Together for Change.

This is a simple six stage process that uses person centred information and themes it to inform strategic change. The person centred information may come from person centred reviews, person centred plans or support plans.

The first day focuses on theming and understanding the information in terms of root causes and success, and the second day develops solutions leading to local and strategic action planning. The working together for change process incorporates co-production principles and the 2 days will involve participants who use services and their families.

The purpose for these two days are for key change agents who have the power to make and influence strategic decisions to:

  • listen to what people are saying, through their person centred information, about their lives and the services they receive.
  • think about the changes necessary to enable people to get more if what is working in their lives and to change what is not working.
  • plan to ensure that the things that people have identified as important to them in the future are incorporated in local and strategic planning.

Who is it for?

Strategic stakeholders and representation from all those involved in self-directed support and workforce development within the field of social care; all other key decision makers, commissioners and influencers, including people from user led organisations and people who use services and their families. Anyone from a senior management level who is involved in local and strategic planning.
