This process is called Working Together for Change, and was developed by HSA, and then rolled out by the Department of Health. Working Together for Change takes information about what is working and not working for individuals, and what is important to them in the future. This information is clustered and themed with people and their families, to co-produce change.
The process is used by providers to inform their business planning and development, and by commissioners to inform their strategic commissioning plans.
