Person centred thinking and planning are being used in nurseries and schools and throughout the transition process. We also worked for two years with Sure Start in the UK to develop person centred approaches.
When, for whatever reason, people are not able to lead the person centred plan themselves, family members are in a great position to develop a plan with and for their family member. This approach is called Families Planning Together.
